2213fa59b5 feat(templates): enforce uniqueness and improve safety 
- Raise TemplateExistsError when duplicate detected
- Migrate placeholder syntax from {} to {{}}
- Add validation for:
  * Reserved keyword collisions
  * Injection attempt patterns
- Update all test cases
